Medi Home Health & Hospice , a proud member of the Medical Services of America family, is currently seeking an experienced Part-Time/PRN Medical Social Worker for our Hospice Patients in Mercer (Mercer County) PA.
Job Description:
· Provides social work services in accordance with the physician’s plan of treatment.
· Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.
· Provides assessment for normal and complicated grief and provides bereavement counseling as appropriate.
· Serves as a liaison with other community resources.
Job Requirements
· Master's level social work degree from a school of social work accredited or approved by the Council of Social Work Education.
· Minimum of one-year Hospice experience preferred.
· Valid Driver's License.
